信用卡直接付款授权书 Credit Card Direct Debit Authorization Form
致 : 润迅通信(香港)有限公司及发出信用卡之银行/公司 To : China Motion Telecom (HK) Limited and the Credit Card Issuing Bank/Company
(1) 本人现授权润迅通信(香港)有限公司【简称「润迅」 】从本人之信用卡账户【简称「信用卡账户」(包括信用卡到期以后)扣除有关本人下述润迅账户内之所有保证金或预 】 付/账单款项【简称「每月应缴款项总额」. 】 I hereby authorize China Motion Telecom (HK) Limited【 ＂China Motion″ 】to charge my card account【 ＂Card Account″ 】(including after the expiry of the card) for all deposits/ advance payments/ bill payments payable 【 ＂the monthly total amount due″ 】to China Motion in relation to the below account(s). 本人同意本人之发卡银行/公司【简称「银行」,按润迅之扣款通知及指示在本人信用卡账户扣除的每月应缴款项总额.此外,银行毋须查证每月应缴款项总额之数额或有 】 关扣款已否通知本人. I agree that my card issuing bank/Company【the ＂Bank″ 】shall debit the monthly total amount due from my Card Account according to China Motion′s debit advice and instructions, and the Bank shall not be obliged to ascertain whether or not notice of the monthly total amount due or any such debit has been notified to me. 本人同意如本人之信用卡账户之信贷额不足以支付应扣款项时,银行将有权自行决定从本人账户上扣除该款项. I agree that should there be insufficient credit available in my Card Account to meet such debit, the Bank shall nonetheless be entitled, in its discretion, to effect such debit. 即使由於扣款造成本人之信用卡账户之结欠账款高於信贷额,本人将自行承担所有账户项下未清缴之款项. I accept full responsibility for all outstanding liabilities under my Card Account notwithstanding the existing limit applicable to my Card Account being exceeded. 本人明白在本授权书上所提供之资料可供润迅(或其供应商及经销商)作提供有关服务及作信用查核,债务追讨及市场调查之用.本人亦明白润迅在提供有关服务时可向 有关人仕及机构包括银行/公司,其他有关服务之供应商或在履行其法律义务时透露本授权书之一切资料. I acknowledge that the information provided by me in this Form may be used by China Motion (or any suppliers or agents) for the purpose of the provision of relevant services and conducting credit checks, debt collections and market surveys. I further acknowledge that China Motion may disclose, relating to the provision of relevant services, such information to others including the Bank, Company or any other services providers or otherwise as and when required by law. 此授权立即生效,直至本人书面通知为止. This authorization shall have effect until it is revoked by me by using the official revocation form of China Motion. 本人不可撤销地同意赔偿润迅因作出上述扣款所受到的一切损失. I hereby irrevocably agree to indemnify China Motion for all losses and damages offered by it in effecting the direct debit herein mentioned.
