Thisisthereportofaninvestigationintothecircumstancesofthedeathofawoman on10May2006atWythenshaweHospital,whilstinthecustodyofHMPStyal. The womanhadbeenfoundhanginginhercell.Shewas42yearsold. Iwouldliketooffermysincerecondolencestoherfamilyandthosetouchedbyher death. InApril2007,Iwaspleasedtomeetpersonallywiththewoman'ssisterand withherlawyer. Theinvestigationwasundertakenbytwoofmycolleagues.BoththeyandIwould liketothanktheGovernorofStyalandtheappointedLiaisonOfficerfortheir kindnessandcooperationduringthecourseofourinquiries.Styalcaresformany prisonerswithahistoryofselfharmandweareawarehowmuchthelossofthis womanwasfeltbyalltheprison'sstaff. ThewomanhadarrivedatStyalon1April2006.Shehadbeenrecalledtoprisonfor breakingherlicenceconditions.Heroriginaloffencewasarson,whichshehadsaid wasasuicideattempt.WhenshewasreceivedatStyal,shehadselfharmedby cuttingherneckandwristinpolicecustody.Shewasaddictedtodrugsandhada longhistoryofmentalhealthproblems.Sheselfharmedseveraltimeswhilein prisoncustody. ThewomanwasonlyinStyalfor40days.Inthattime,shereceivedagreatdealof supportandcareforhermanyproblems.Iamimpressedbythecommitmentand compassionshownbystaff.However,Ihavefoundareaswithroomfor improvement,especiallyinrespectofthethesupportthementalhealthinreach teamgave.MyreportalsodrawsattentiontothedemandsplaceduponStyal's managementandstaff,giventhevulnerabilityoftheprisonersforwhomtheycare. . Imakeatotalof11recommendationsandhighlighttwoareasofgoodpractice. I wasparticularlyconcernedtolearnthatstaffatStyalwerenotconfidentintheiruse ofdefibrillators.Theseareliterallylifesavingmachines,andthePrisonServicemay wishtoconsiderifsufficientdisciplineandhealthcarestaffaretrainedintheiruse. Allmyreportsareaboutthedeathsofindividuals.However,Iamveryconscious thatthiswomanwasalltoorepresentativeoftheverymanydamagedandvulnerable womenwhoendupinStyalandtheotherwomen'sprisons.Iplayedsomerolein assistingBaronessCorstonduringheryearlongreviewofwomenwithparticular vulnerabilitiesinthecriminaljusticesystem,areviewthatgrewoutofpublicconcern aboutaseriesofsixdeathsofwomenatStyalina13monthperiodin20022003. MuchofthefocusoftheCorstonreportwasontheneedsofwomenwithmental healthanddrugproblemsandthedevelopmentofalternative,moretherapeutic, morewomencentredapproachestotheirtreatmentandrehabilitation. Thesad deathofthiswomanservestoemphasisetheimportanceofBaronessCorston's findingsandrecommendations.
