English Please give name of the company/registered business as shown on the Business Registration Certificate. Please give full address, tel. and fax numbers of the head office (and branch office, if any) of the company/registered business as appropriate. Email address is optional. Please give full address of the storage place for which approval is applied, and its storage capacity. It is a condition of approval that a rice storage place must immediately inform the Director-General of Trade and Industry (DGTI) in writing of any change of particulars provided in this application form . Accordingly, owners/operators of such storage places who wish to store rice above the approved capacity of a storage place must first apply for and obtain the DGTI's approval for enlarging the capacity of their storage place. Failure to do so may constitute a failure in complying with Condition 1 of the "Conditions of Approval as a Rice Storage Place", which may result in DGTI's withdrawal of his approval granted under Regulation 10 of the Reserved Commodities (Control of Imports, Exports and Reserve Stocks) Regulations (Cap. 296) to the storage place in question. The person who makes the declaration and signs this application must be: (a) for sole proprietorship, the proprietor; (b) for partnership, one of the partners; or (c) for limited company, a director or a responsible person authorized by the board of directors (a letter of authorization is required to be provided in the latter case).
